The slope intercept formula is used to generate the equation of a line using its slope and intercept points. We do so by using the formula:
y = mx + b
Where:
m = the slope of the line
b = the y-intercept
You can calculate m by using the coordinates of two points on the line and the Slope Formula: y2-y1

You can calculate b by looking at the qraphed line, and seeing which point that line crosses the y-axis directly.
